PerformCare: An Overview of the NJ Children's System of Care IDD Services

PerformCare and the NJ's Children's System of Care (CSOC) provide services to individuals up to the age of 21 who have intellectual and/or developmental disability (I/DD), behavioral health, or substance abuse needs.

This program is designed to provide information about I/DD services available through CSOC and to support parents, families, and caregivers through the Developmental Disability (DD) Eligibility Application process.

PerformCare representatives will discuss:
  • CSOC I/DD services such as Family Support Services (FSS), respite, summer camp, etc.
  • Eligibility requirements for I/DD services
  • DD Eligibility application process
  • When and how to contact PerformCare
