OARC provides support, resources, referrals, system navigation, & helps develop a plan of care. OARC encourages self-empowerment & self-sufficiency by setting up individualized services, goals, and interventions needed to address a person's specific health, wellness, or social needs. OARC helps ensure that services are coordinated, comprehensive, and focused on the individual’s unique circumstances, which serves as a roadmap for achieving positive outcomes by addressing medical, psychological, and social aspects of care.  OARC provides services in English & Spanish. Eligibility: Seniors (60 years old +) OR individuals living with a disabling condition, residing in Ocean County, who have been impacted or experienced hardship due to COVID19 pandemic.
