The leap from person-centered thinking to person-centered planning can seem daunting and abstract. The prospect of accessing services through the adult service system can seem overwhelming and often requires a great deal of advocacy and persistence. Consequently, it is easy for an individual and their support team to fall into the trap of centering eligibility-based supports rather than ensuring that the vision for a meaningful life is guiding each and every step.
Part 2 will discuss how to put the plan into action, including how to survey what is available within community as well as exploring eligibility-based supports. Participants will get an overview of the eligibility process and the services available through the Department of Developmental Disabilities (DDD) as well as the role of the support coordinator. Participants will consider how potential services may be utilized in concert with community resources to help plan and create a full and meaningful life.
